Interested Consultancy firms/Individual should submit their technical and financial proposals in a seald envelope which includes.
1. Copy of renwed/recent professional and/or business license.
2. Copy of Taxpayers and VAT Certificate.
3. TIN Certificate
4. Staff compostion and firm profile including principl partner.
5. Previous Consultancy experince with International NOG
6. Estimated Time frame and othe relvant information for the consultancy work
Deliverables
- Selected site mapping document for date collection with dfinal check by MEAL Manager
- Enumerator training presentaion detailing operational definition of terms special cases and standardized date collection procedures.
- Completed and signed consent forms from all health facilites/responents.
- Raw date set in Excel with the agreed upon number of completed interviews per the sampling methodology.
- Both Technical and Financial proposal /clearly indicate with or without VAT/ shoulde be submitted to Medical Teams International Ethiopia Head office starting from Wednesday July 19,2023 to August 1,2023 to the address below/
- Bid offer should be hand delivered and placed in a ''Tender Box'' at MTI-office Logistics and Procurement unit.
- ITTs wll be opend on 2nd August 2023 at 9:00 AM East Africa Time at Meedical Teams Interantional Office by MTI Procurement committee memebers without the presence of bidders or their legal representatives
- Medical Teams International Ethiopia reseves the right to accept or riject the ITT partially or flly
